The term carer is used in this survey to capture any family, friend caregiver, support person or Substitute Decision Maker (SDM).

Your feedback as a carer is valuable to us because your responses are used to inform the improvements we can continue to make to improve patient experiences across our hospital services. Your responses and comments will be treated confidentially.
